This is an advert for a person to be the co-ordinator for falls prevention within the Single Point of Access (SPOA) in Conwy. To act as a link between health, social care, ambulance service, the independent care sector and third sector partners in Conwy
The post will involve the implementation of the Falls Risk Assessment Tool (FRAT) within the SPOA and the associated Multifactorial Risk Assessment (MRA) within Conwy face to face with service users, working to mainstream this approach and embed it within the locality model. In doing this you will be supporting people to remain independent, promoting wellbeing and resilience within the community and their own homes.
The successful candidate will co-ordinate all Falls Prevention referrals into the Single Point of Access, completing multifactorial risk assessments (MRA), advising about training and work together with the training instructors.
